I think I fixed it. In the BOM it gives the name of R1, R2, R3 as a 270 ohm resistor but a part number for 270k ohm. It should be 270 ohm. I switched out the resistors and the relays work now.

I added the heat sink and that seems to help. Heat sink gets too hot to touch, but no crashing. Probably would be a good idea to add a heat sink to the BOM.
